How to Apply DermoNeuroModulating (DNM) Techniques

This page introduces how to apply DNM techniques in practice, using the core principles and clinical methods from the Pinpoint DNM Treatment Techniques Manual, adapted from the original DermoNeuroModulating book by Diane Jacobs, PT (with her permission).

DNM Overview

DermoNeuroModulating (DNM) is a gentle, science-based manual therapy approach that uses the skin as a direct interface with the nervous system.

It was developed by Diane Jacobs, PT, and has gained global recognition for its clinical effectiveness in working with complex pain conditions, especially when traditional structural or deep tissue techniques fall short.

Unlike traditional manual therapies that aim to “fix” tissues, DNM invites the body to self-regulate through calm, precise skin movement.

PinpointDNM continues this evolution of DermoNeuroModulation, offering a uniquely structured, practice-forward training system that brings clarity to clinicians and real relief to patients.

What Is DNM in Practice?

DNM is applied through specific, intentional skin-stretch techniques designed to modulate nervous system activity.

Instead of targeting muscles, joints, or fascia directly, DNM interacts with the cutaneous nerves in the skin to send non-threatening, novel input throughout the nervous system.

This well-researched phenomenon helps reduce protective output (like pain or muscle guarding) and promote a feeling of safety, and therefore, a calmer internal state.

Key Application Concepts for DNM

Pain is an experience and a nervous system phenomenon.

  • Touch should be innocuous and receptive, not forceful.
  • Safe, calming, non-invasive contact
  • Working only with the skin, and not “pressing” into deeper tissues
  • Skin movement adapts to follow the cutaneous nerve distribution.
  • Shortening and widening the neural tunnel
  • Introduce novel, non-noxious input to down-regulate the nervous system.
  • Following the “less is more” principle
  • Let the nervous system self-correct without rushing it.

DNM is particularly effective for tunnel syndromes, nerve irritations, and chronic or widespread pain. It’s also well-tolerated by sensitive patients, making it ideal for those who can’t handle aggressive manual therapy, and shouldn’t have to.

The Pinpoint DNM 4 Phase Clinical Approach for DermoNeuroModulating

1. BioPsychoSocial (BPS) Intake & Assessment

  • Establish a therapeutic alliance: listen, reflect, and ask.
  • Provide informed consent and relevant pain neuroscience education (PNE).
  • Assess movement limitations or pain triggers.

2. DNM Manual Therapy Application

  • Ensure comfort first: adjust temperature, bolsters, lighting.
  • Use dry, warm hands that naturally “stick” to the skin.
  • Apply techniques gently, gradually, and slowly.
  • Prioritize body positions that slacken nerves.

3. Reassessment During & After Treatment

  • Retest the original painful or restricted movements.
  • Ask for patient feedback.
  • Modify treatment based on response, with efficiency.

4. Movement Homework

  • Assign 2-3 minutes of gentle, simple movement.
  • Encourage continued post-session self-care.
  • Follow up and reinforce self-efficacy.

The 6 DNM Skin-Stretch Techniques

Illustration of the 6 key DermoNeuroModulating (DNM) techniques taught in the PinpointDNM system, including Longitudinal Distraction, Unloading, Shearing Distraction, Rotational Shearing, Contralateral Unloading, and Circumferential Unloading, adapted from Diane Jacobs’ original DNM work.
Included in Pinpoint DNM Part 1: Foundations
  1. Longitudinal Distraction – gentle opposing stretch along a line
  2. Unloading– approximating tissue around a sore point
  3. Shearing Distraction– opposing parallel slides
  4. Rotational Shearing– skin stretch in opposite rotational directions
  5. Ballooning Technique– contralateral unloading to open space
  6. Circumferential Unloading– multidirectional opening/stretch

These techniques invite change through minimal effective contact and slow progression. Aim for softness and ease. Pain or discomfort should never increase.

The Pinpoint DNM Difference

DNM can be explored in many ways, but Pinpoint DNM was meticulously designed to help practioners truly master it and gain more and more confidence in taking on challenging cases not usually addressed with massage or manual therapy.

Developed over nearly a decade of dedicated teaching, testing, meticulous refinement, and real-world application, this unique system helps you not only deeply understand DNM, but also apply it confidently, especially with complex pain cases.

Why Pinpoint DNM?

Based on years of feedback, it seems many people hit a wall with DNM after the learning basics or don’t have the lesser known tools to unlock its full potential and tackle more challenging cases with confidence.

They aren’t sure how to apply it to unfamiliar pain presentations or how to maintain the subtlety and patience the method demands.

Learn Efficiently

I know because I’ve been there, I’ve struggled, and figured out the exact, reproducible strategies it takes to push through it, and I’ve successfully taught many others, including beginners to do it, too!

You can now bypass those common obstacles. That’s where Pinpoint DNM stands out.

You don’t just “learn” DNM here. You learn how to keep learning DNM while you practice it. That’s what builds confidence, deep understanding, and mounting clinical results.

Frequently Asked Questions

What is the difference between DNM and other manual therapy methods?

Unlike deep tissue or structural approaches, DNM works at the skin level to influence the nervous system. It focuses on safety, subtlety, and neuro-modulation rather than mechanical change.

Why do some practitioners struggle to apply DNM after initial training?

Many DNM learners plateau because they lack a structured system, clinical context, or ongoing support. It’s normal to default to what we already know, but that’s exactly what Pinpoint DNM was designed to address.

By providing tested, replicable, and accessible methods and tools for real-world success, it makes DNM an approach you’re empowered to implement right away.
